4. Certificate of Premium Contribution signed by Authorized Representative of Company

Individually Paying:

1. MDR (photocopy)

2. Philhealth ID (photocopy) 

3. CF1

4. Photocopy of Premium Contribution (at least 3 months of premium contribution prior to the date of surgical/laser procedure.)
